Where I can find my medical records?
Hello. I wondered how I could go get my own medical history. I a car accident while doing my lawyer told me I have to send in my old medical records. The problem is about 10 years between when my family moved to the area Chicago, and the time I left, I can only remember going to the doctor twice. And I do not remember the name of one of these doctors. Is there a central database with more of my medical records? I remember the location of doctors, but both were in large practices, and I think all of them were independent. I do not know no who to call. any help would be appreciated. Thanks!
